Information collected
The only personal information collected by this website is the information you provide voluntarily through our contact forms:
- Full name
- Phone number
- Email address (when provided)
- The service you are interested in and, in a few words, the reason for your request (when provided)
- For facial aesthetics, your situation (first consultation or not) and, for the “Contact us” form, the best time to call you back (when provided)
Detailed health information (medical history, medications, allergies) is not requested online. It is collected through the new patient questionnaire, which you fill out on screen or on paper and hand in to the clinic.
Purpose of the collection
Your information is used only to contact you back to book an appointment or to answer your request for information. It is never sold, rented or exchanged for commercial purposes.
Recipients and authorized technical providers
To operate the website and process requests, some information may be processed by authorized technical providers, only within the scope of their mandate:
- Formspree — processing and forwarding of contact forms by email.
- Netlify — hosting and delivery of the website.
- The clinic's email service — receipt of requests forwarded by Formspree.
These providers only have access to your information to the extent strictly necessary to perform their services and do not use it for any other purpose.
Hosting and transfers outside Quebec
Because third-party technical services are used, some information may be processed or hosted outside Quebec or Canada. Reasonable safeguards are in place to ensure a level of protection equivalent to that required by Quebec law.
Retention of information
Information sent through the forms is kept only as long as needed to process your request, according to the following criteria:
- Web requests with no follow-up: deleted within a reasonable time after processing, generally within 12 months.
- Requests that lead to opening a patient file: kept in accordance with the professional obligations of the Ordre des dentistes du Québec.
Security measures
Reasonable technical and organizational measures are in place to protect your personal information against loss, unauthorized access, disclosure, alteration or destruction. Forms are transmitted in encrypted form (TLS/HTTPS).
Cookies and tracking
This website uses cookies that are strictly necessary for its operation, for example to remember your choice regarding the consent banner. It may also use Google Analytics 4, an audience measurement tool, only if you consent through the banner shown on your first visit. The website also counts its visits with Cloudflare Web Analytics, which sets no cookie, assigns no identifier and does not follow you from one visit to the next: only the number of page views, the country and the referring page are measured. It also uses Umami, an audience measurement tool whose data is hosted in the European Union: without cookies and without a persistent identifier, it measures page views, the referring page, the length of the visit and the approximate location (country, region, city). This location is estimated from the IP address, which is not kept. If you decline Google Analytics, no other browsing data is collected. You can withdraw your consent at any time by deleting the cookies in your browser. No advertising retargeting pixel is installed and your browsing is not tracked for commercial or marketing purposes.
Forms and data processing
The forms on this website are processed by the Formspree service. Submitted data is transmitted securely (TLS encryption), then sent by email to the clinic. Formspree may also keep a copy in a secure space reserved for the clinic, deleted according to the retention periods above. For more information, see Formspree's privacy policy.
Your rights
Under Quebec's Act respecting the protection of personal information in the private sector, you have the right to:
- Access the personal information we hold about you;
- Request the correction of information that is inaccurate, incomplete or ambiguous;
- Request the deletion of your personal information;
- Withdraw your consent to the processing of your information, subject to any legal or professional obligations that may apply.
To exercise these rights, please contact us at 450-777-0373 or through the form.
Right to file a complaint
If you believe your rights regarding the protection of personal information have not been respected, you may contact the Commission d'accès à l'information du Québec.
